I had problems with the links. It takes me to a random site that asked me to download the information which looked like a virus. I also have a flier that said to following the following link for an application, taylorsvilleut.gov/arts.main.html. There was no application at that site.
I had the same problem. This is the website that the 2 links connect to: http://www.freefilehosting.net/ Would it be possible to have the forms emailed to me, or would I be able to pick them up from City Hall?
